New Student Registation Process & FORMS  

We are excited that you have chosen to register your child(ren) at London Towne Academy!  The registration timeline for 2024 is noted on our HOME PAGE. Please contact the office directly for a hard copy of current registration forms.  Along with the registration form, we ask that you print out and/or complete the following:     





  • Registration Form/Parent Agreement Form & Permission Slip (CALL OFFICE TO OBTAIN)

    • Please be as detailed as possible in your requests and turn in forms with the necessary fees.  

    • Initial and sign where indicated on the Parent Agreement Form and Permission Slip.

    • You will receive a signed copy of this form or a confirmation letter/e-mail once your child has officially been registered in our program.

  • Emergency Form

    • This form will be updated annually in case of any changes/additions. 

    • We require that (at least) one emergency contact be listed.

  • Health Inventory

  • Topical Basic Care Product Application Authorization Form - MUST be completed for sunscreen/bug spray!!!

  • Registration Fee &/or Activity Fee

    • Noted on registration form 

  • Please turn in ALL of the necessary paperwork/fees noted above at the same time. 

  • The registration process will not be complete until we have received ALL things noted above.
